Through the Priority Health Quality Award for Exceptional Patient Care,
the company is recognizing more than 200 Michigan primary care practices
for their excellence in clinical quality measures based on national
standards and solid, evidence-based medicine. The program is designed to
reward primary care providers for helping ensure patients receive
preventive care and manage chronic conditions while demonstrating
patient-centered medical home practices to support the delivery of
high-quality and cost-effective care. Each award winner is selected by
analyzing the results of the Priority Health Primary Care Provider
Incentive Program.
One example of the program’s success is demonstrated through chronic
disease care. By collaborating with physicians, Priority Health has
successfully helped health plan members avoid incidence of chronic
disease complications for conditions like congestive heart failure,
chronic obstructive pulmonary disease, diabetes mellitus, asthma and
hypertension – at rates better than state and national averages.

Since 2003, when the Quality Award was established, Priority Health has
paid $150 million in support of primary care, over and above standard

in 2013, based on their performance in 2012.

provided by Priority Health, that offer details on a patient’s history,
